An intentional private and partnership services for senior care or home health agencies. It’s A Heart Thang acts as a heart-centered extension of their services — focusing on (activities of daily living) social wellness, emotional vitality, and community inclusion for adults. HEART Link builds bridges that focus on thrive in and outside their homes. Companion /Engagement • One-on-one social visits (conversation, reading, reminiscing, walks) • Emotional presence during transitions or end-of-life phases • Non-medical support only 2. Socialization Through Community • Encouraging seniors to attend: • neighborhood events • outreach programs • wellness gatherings • Escort support when possible (insurance-aligned activities) 3. Connection to Existing Resources • Partnering with: • hospice agencies • neighborhood outreach events • senior-focused programming already happening • Acting as the bridge, not the provider of everything
